Drug intelligence / Profile preview
4-[18F]fluoro-4-deoxyfructose (NeuCaVis) is a first-in-class, small molecule radiopharmaceutical PET imaging tracer developed by Yellowbird Diagnostics in collaboration with the University of Ottawa and the University of Ottawa Heart Institute. It is a radiolabeled fructose analog designed to visualize active inflammation and cancer by targeting fructose metabolism (fructolysis). Unlike glucose-based tracers like [18F]FDG, which exhibit high background uptake in healthy brain and heart tissues, 4-[18F]fluoro-4-deoxyfructose shows minimal background signal in healthy tissues because they utilize very little fructose. Upon entering cells via the fructose transporter GLUT5 (SLC2A5), it is phosphorylated by ketohexokinase (KHK) to form 4-fluoro-4-deoxyfructose-1-phosphate. The C4-fluorination prevents aldolase-mediated cleavage, trapping the radiolabeled metabolite intracellularly. It is currently in Phase I clinical development for imaging neuroinflammation, cardioinflammation, and certain cancers such as lymphoblastic T-cell lymphoma.
